    University offering free flu shot clinics on campus for benefits-eligible employees


    The University has established a program to provide free flu shots at clinics at several campus locations to active, benefits-eligible University employees.

    No appointment is necessary, but an appointment can be scheduled with Occupational Medicine by calling (773) 702-6757.

    Eligible University employees should bring a University I.D. card and a completed “Influenza Vaccine Record 2006-07” to the vaccination site. This form is available at http://adminet.uchicago.edu/adminann/uhrm_ann/flu-shot.shtml, and there will be forms available at the clinic locations.

    The flu shot clinics are from 1 to 3 p.m., and the dates and locations are as follows:

    Friday, Nov. 3, School of Social Service Administration, Lobby, 969 E. 60th St.

    Monday, Nov. 6, and Wednesday, Nov. 8, Joseph Regenstein Library, Room 127, 1100 E. 57th St.

    Friday, Nov. 10, Graduate School of Business Hyde Park Center, Room 104, 5807 S. Woodlawn Ave.

    Due to a limited vaccine supply, this program will not be extended to retirees, family members, temporary employees, University Human Resources Management temporary pool employees or students. University students should contact the Student Care Center at (773) 702-4156 or visit its Web site at http://scc.uchicago.edu for information on the free flu shot program for students.

    The SCC is located at 5839 S. Maryland Ave., Room 100.
